I _________________ hereby agree and consent to the following.

I consent and authorize street footie LLC to use my likeness in any photography, video, or digital media (“Photos”) taken or to be taken in any and all its publications, including print or web-based publications.

I irrevocably authorize street footie LLC to copy, edit, enhance, crop, or otherwise alter any photography, video, or digital media (“Photos”) for use in their publications.
I also waive any rights for approval or inspection of any media.

I understand and agree that all photography, video, or digital media (“Photos”) are the property of street footie LLC and will not return to me.

I acknowledge that I am not entitled to any compensation or royalties with respect to the use of the photography, video, or digital media (“Photos”).

I agree to release and forever discharge street footie LLC and its affiliates, successors and assigns, officers, employees, representatives, parents, agents, and anyone claiming through them, their individual and/or corporate capacities from any and all claims, liabilities, obligations, promises, agreements, disputes, demands, damages, causes of action of any nature or kind, known or unknown, which I and anyone claim on behalf of me, may have or claim to have against Release in connection with this Release.
